Vascular ultrasonography for follow-up of endovascular repair of abdominal aorta aneurysms
Conclusions Vascular ultrasonography is an excellent primary method for evaluation and post-surgical follow-up of patients treated with endovascular repair of abdominal aorta aneurysms. However, when ultrasonography detects problems or is difficult to accomplish, it should be supplemented with a more specific investigation for diagnostic confirmation.

Brachial index does not reflect upper extremity functionality following surgery for vascular trauma
CONCLUSIONS: Evaluation of the postoperative outcomes of this small series of patients with penetrating vascular traumas in the upper extremity revealed that 15% of them suffered occlusion of one artery of the upper extremity. Artery occlusion did not correlate with brachial-brachial Doppler index, probably due to rich collateral circulation, but occlusion was associated with an extremity that was dysfunctional for the purposes of daily work. The result of the brachial-brachial index does not therefore correlate with functionality. OBJETIVOS: Os vasos das extremidades superiores são frequentemente lesados em razão de aci...

Trends of vascular surgery procedures in Marfan syndrome and Ehlers-Danlos syndrome.
CONCLUSIONS: Patients with Marfan syndrome and Ehlers-Danlos syndrome differ from other patients undergoing vascular surgical procedures, with a significantly higher proportion of aortic procedures including open aneurysm repair and thoracic endovascular aortic repair. While they are younger with fewer comorbidities, due to the unique pathogenesis of their underlying connective tissue disorder, there is an overall higher rate of procedural complications and increased length of stay and cost for Marfan syndrome patients undergoing aortic surgery. Setting high-impact clinical research priorities for the Society for Vascular Surgery
With the overall goal of enhancing the effectiveness and efficiency of vascular care, the Society for Vascular Surgery (SVS) recently completed a process by which it identified its top clinical research priorities to address critical gaps in knowledge guiding practitioners in prevention and treatment of vascular disease. After a survey of the SVS membership, a panel of SVS committee members and opinion leaders considered 53 distinct research questions through a structured process that resulted in identification of nine clinical issues that were felt to merit immediate attention by vascular investigators and external fundin...

Variation in smoking cessation after vascular operations
Objective: Smoking is the most important modifiable risk factor for patients with vascular disease. The purpose of this study was to examine smoking cessation rates after vascular procedures and delineate factors predictive of postoperative smoking cessation. Methods: The Vascular Study Group of New England registry was used to analyze smoking status preoperatively and at 1 year after carotid endarterectomy, carotid artery stenting, lower extremity bypass, and open and endovascular abdominal aortic aneurysm repair between 2003 and 2009. Of 10,734 surviving patients after one of these procedures, 1755 (16%) were lost to fol...

Approach to the vascular complications of lumbar disc surgery.
The objective of this study was to demonstrate the role of major surgery on patients presenting with vascular complications after lumbar disc surgery. A retrospective analysis of seven cases treated surgically in two tertiary care centers between August 2001 and June 2010 was carried out. The average age of patients (three women and four men) was 35.8 ± 7.2. The most common vessel injured was the left common iliac artery occurring in five patients (71.4%), followed by the left common iliac vein injury detected in two patients (28.5%). Transperitoneal approach was preferred in all cases and primary suturing, graft interpos...

Recent trends in publications of US vascular surgery program directors.
Conclusion There seems to be a significant decline in the number of publications over the last 15 years. Yet, the subject of the publications has progressed from Open to TEVAR with an increase in endovascular publications. However, basic science publications reduced by half. A Vascular Surgeon Plays a Critical Role in the Development of a Successful TAVR Program
Transcatheter aortic valve replacement (TAVR) is a Food and Drug Administration-approved means of treating symptomatic aortic stenosis in high-risk patients. A number of techniques to deliver valve prostheses are currently in practice, including transfemoral (TF), transapical, and transaortic approaches. Various disciplines are involved throughout this complex process. Our team includes a vascular surgeon, bringing greater experience in aortoiliac imaging, vascular access, and repair. We analyzed the role of a vascular surgeon during the selection process and throughout safe device delivery.

Transradial interventions in contemporary vascular surgery practice.
Conclusions Using the transradial approach, we have demonstrated a high technical success rate over a range of clinical contexts with minimal morbidity and no significant complications such as bleeding or hand ischemia. The safety profile compares favorably to historical complication rates from brachial access. Radial access is a safe and useful skill for vascular surgeons to master. Stroke rate after endovascular aortic interventions in the Society for Vascular Surgery Vascular Quality Initiative
The stroke rate after endovascular aneurysm repair (EVAR), particularly complex EVAR such as fenestrated EVAR (FEVAR) and chimney EVAR (chEVAR), is not well defined. Whereas stroke is a well-established risk of thoracic endovascular aortic repair (TEVAR), the impact of procedural characteristics on stroke remains unclear. Therefore, we characterized the risk of stroke after endovascular aortic interventions in the Vascular Quality Initiative database and identified procedural characteristics associated with stroke.

Gender differences in abdominal aortic aneurysm presentation, repair, and mortality in the Vascular Study Group of New England
Objective: Prior studies of gender differences in abdominal aortic aneurysm (AAA) repair suggest there may be differences in presentation, suitability for endovascular aneurysm repair (EVAR), and outcomes between men and women. Methods: We used the Vascular Study Group of New England database to identify all patients undergoing EVAR or open AAA repair. We analyzed demographics, comorbidities, and procedural, and perioperative data. Results were compared using the Fisher exact test and the Student t-test. Multivariable logistic regression and Cox proportional hazards modeling were performed to identify predictors of mortalit...

The effect of postoperative myocardial ischemia on long-term survival after vascular surgery
Introduction: The impact of a postoperative troponin elevation on long-term survival after vascular surgery is not well-defined. We hypothesize that a postoperative troponin elevation is associated with significantly reduced long-term survival. Methods: The Vascular Study Group of New England registry identified all patients who underwent carotid revascularization, open abdominal aortic aneurysm repair (AAA), endovascular AAA repair, or infrainguinal lower extremity bypass (2003-2011). The association of postoperative troponin elevation and myocardial infarction (MI) with 5-year survival was evaluated. Multivariable models ...

Isolated Iliac Vascular Injuries: Morbidity of Repair Versus Ligation of Iliac Vein Injury
The morbidity and mortality for iliac vascular injuries in the literature are likely to be overestimated because of associated injuries, and data for isolated iliac vascular injuries are limited. No large studies have reported the incidence of morbidity for repair vs ligation of isolated iliac vein injuries. Novel endovascular therapies have refocused attention on the algorithm for pelvic trauma, and balloon occlusion of the aorta may allow more expeditious repair of arterial injuries.
